Immunization Requirements
What are the mandatory immunization requirements?
By New York State law, any students born on or after January 1, 1957, attending a public college or university, and registered for six (6) or more credits with any credits on campus (“on campus” is defined by NYS as traditional, blended, and hyflex classes; clinicals; and classes held at affiliated sites), must submit proof of immunizations/immunity for Measles, Mumps, and Rubella (MMR) and for Meningococcal/Meningitis. SUNY Broome must have on file:
For Measles, Mumps, and Rubella (MMR) one of the following:
- Proof of two (2) MMR (combined Measles, Mumps, and Rubella) vaccines given at least 28 days apart after your first birthday. (Students who are in process of obtaining their MMR vaccinations have 28 days from the first dose until the second dose is due. Students missing the 2nd dose will be excluded until compliant.)
- Proof of individual vaccinations for Measles (2 vaccinations given at least 28 days apart), Mumps (1), and Rubella (1) given after your first birthday
- TITER blood lab results showing antibody levels (numbers must be included on the lab results) indicating immunity to all three diseases
For Meningococcal/Meningitis one of the following:
- Proof of one (1) vaccination received within the last 5 years
- A completed and signed Meningococcal Meningitis Vaccination Response/Waiver form. If you fill this form out electronically, it must contain an electronic signature.
When do I need to submit my records by?
Complete records must be on file with Student Health Services before the start of your first attending semester. Those students without complete records will be medically withdrawn from classes and be responsible for all financial obligations.

Where and how can I submit my records?
You, your provider, or a previous school can submit your records or Certificate of Immunization by:

Not sure where to get your immunization records?
All records must have your full name displayed as part of the record. It is preferred that your date of birth is also displayed.
- High school health office
- Primary care physician
- Previous college/university
- Military health records
- Childhood health record book
- Online health records i.e, Mychart
Religious Exemption for MMR Only
Persons who hold genuine and sincere religious beliefs contrary to immunization may apply for Religious Exemption. Philosophical, political, scientific, or sociological objections to immunization is not accepted by the State of New York as a religious exemption. Further documentation/supporting evidence may be requested by the College when submitting Religious Exemption.
To request a vaccine religious exemption, submit the Vaccine Religious Exemption Request Form.
Meningitis Waivers
To submit a Meningitis Waiver, fill out, sign, and return page 2 only of the Certificate of Immunization and Meningococcal Meningitis vaccination Response/Waiver form. If you fill the form out electronically, you must use an electronic signature in order for it to be acceptable.
Consequences of Being Non-Compliant
Non-compliance will result in holds being placed on your student account and will lead to deregistration from classes. Students are responsible for any fine incurred by the college for non-compliance and for any financial obligations for deregistration.
